
More than two dozen Chicago area hospitals earned “A” grades in hospital watchdog group Leapfrog’s latest Hospital Safety Grade report spring 2026, with one earning a “F” grade and others moving up in the rankings.
The biannual safety report, released Wednesday, assigns grades to most general hospitals in the U.S. “based on their ability to protect patients from medical errors, accidents, injuries and infections.” According to the report, thousands of hospitals that participated in Leapfrog’s most recent hospital survey received grades using what the group called 32 “evidence-based measures” of patient safety.
The grades range from “A” to “F,” with a total of 108 hospitals in Illinois receiving a grade. Roseland Community Hospital — a Safety Net hospital in Chicago’s West Roseland neighborhood — received an “F.”
In a statement, hospital CEO Tim Egan called the Leapfrog report “unfair,” describing it as a “pay to succeed process,” and citing a recent ruling by a court in Florida against the group.
“Leapfrog’s fallible grading system has been successfully challenged legally and a Federal Judge ruled Leapfrog uses unfair and deceptive practices,” the statement said. “Once again, Roseland disputes this chamber of commerce pay to succeed process. We do not have the resources to commit to playing Leapfrog’s game and we believe the process is prejudiced toward struggling Safety Net Hospitals.”
According to Leapfrog’s report, the group is “pursuing an appeal and reviewing the Safety Grade methodology Grade methodology with its National Expert Panel to ensure the Hospital Safety Grade continues to be useful for consumers.”
Mount Sinai Hospital, at 1500 South Fairfield Ave. in Chicago, earned a “D” grade.
A statement from Mount Sinai Hospital said the medical center is committed to patient safety, and that its grade reflects on data from the two years prior rather than on recent changes.
“The Mount Sinai Hospital grade reflects a prior period and does not capture the significant progress we’ve made over the past year, including measurable reductions across key safety and quality metrics and improvements in patient communication,” the hospital’s statement read in part. “In the time since the data used in the current grades was collected, we have made key leadership changes focused on safety and quality, have strengthened our daily patient safety huddles and implemented quality and patient safety forums to engage our frontline caregivers.”
A number of hospitals from several large systems in the Chicago area, including Advocate; Northwestern; Loyola; and Prime Healthcare, which now owns eight Ascension Illinois hospitals, earned “C” grades.

The largest majority of hospitals in the Chicago area earned “B” and “A” grades, including all seven of Endeavor Hospitals. One of “A” hospitals, the University of Illinois Hospital and Health Sciences System in Chicago, moved up from a “C” grade from the fall.
“This recognition reflects years of deliberate work by our physicians, nurses, advanced practice clinicians, staff and leaders to strengthen the systems that keep patients safe,” Dr. Sagar Harwani, chief quality officer of the University of Illinois Hospital & Clinics, said in a release. “An ‘A’ from Leapfrog is meaningful because it is based on transparent, evidence-based patient safety measures. We are proud of this progress, and we recognize that patient safety requires continued focus, discipline and improvement.”
